ISDA Conferences COVID-19 Safety Acknowledgment Form – ISDA Crypto Forum 2022 Liability Waiver and Release of Claims Event Name: ISDA Crypto Forum 2022, Wednesday, October 26, 2022 The International Swaps and Derivatives Association Inc. (ISDA) looks forward to welcoming you back to its in-person conferences. Due to the extremely contagious nature of COVID-19, ISDA may put in place preventative measures and precautions pursuant to guidelines from the U.S. Center for Disease Control (CDC) and the United Kingdom's Department of Health and Social Care (NHS), as applicable, and local ordinance and regulations. Such measures may include, among others, requiring "social distancing" to be maintained or the use of face coverings. However, these measures cannot guarantee that participants in ISDA's in-person conferences will not become infected with COVID-19. For the safety of ISDA's guests and employees, you must attest that you WILL NOT attend an ISDA in-person conference if: 1. You experience, within 5 days of the event start date, any symptoms of COVID-19, which include fever, cough, shortness of breath or others as specified by the CDC; 2. You are diagnosed with COVID-19 and have not yet received a test that you have been cleared as non-contagious by applicable health standards; 3. You qualify for any of the restrictions specified by the CDC or NHS, which are updated from time to time. 4. Local government and venue safety requirements may change between the date of registration and the date of the event. ISDA reserves the right to require attendees to follow additional health and safety measures as required by local law, the venue, or as it deems prudent. RELEASE AND WAIVER I acknowledge that I receive educational and other benefits from participating in the ISDA in-person conferences, and I willingly engage in this event (the “Conference”).
