This is to confirm that the undersigned has agreed to participate in the International Swaps and Derivatives Association, Inc. conference described above.

As part of its continuing education efforts, ISDA occasionally records its programs and makes those audio and video recordings, and/or edited transcriptions thereof, available to those interested in the topics being covered. We are therefore requesting your permission to record, edit, transcribe, duplicate, distribute, and perform your presentation delivered at this program, in any and all media now existing or hereafter developed, throughout the world.

As a condition of publication and for no monetary compensation, we further request the nonexclusive worldwide rights to reproduce, distribute, and sell any written or visual material submitted in connection with your oral presentation, in whole or in part, in any media, as part of a course book or any other publication published under the auspices of ISDA and to license these rights to others. We shall have the right to edit a transcript of your presentation.

You also grant ISDA the right to use your name and voice, and, if we so choose, your photograph and biography, in connection with the presentation and the other materials.

You warrant that your presentation and any other material you submit are original with you, that their publication will not infringe on the rights of others, and that you have full power to grant this license. Should your presentation or other materials require permission for use by ISDA, you agree to obtain that permission from the copyright proprietor prior to use consistent with this Agreement.

If the foregoing terms are satisfactory, please complete this agreement below and submit it to the ISDA Conference Department. Execution of this Agreement does not obligate ISDA to publish your presentation or other materials.
